The Medal of Honor is the United Commonwealth of America Armed Forces' highest decoration, currently awarded in the Antilles to soldiers, sailors, coast guardsmen, airmen, national guardsmen, and marines for acts of extreme bravery and/or outstanding military leadership in the field. The medal is nominally awarded by the National Assembly of the United Commonwealth (formerly the United States Congress before 1868), though in practice by the president of the United Commonwealth, which is why the medal is also traditionally nicknamed the "Congressional Medal of Honor".

There are three variants of the Medal of Honor: the Department of the Army variant, for soldiers and national guardsmen; the Department of the Navy variant, for sailors, marines, and coast guardsmen; and the Department of the Air Force variant, for airmen and air national guardsmen. It is the oldest continuously issued decoration in the U.C. Armed Forces, with the Navy first receiving its version in 1861, after the outbreak of the American Civil War, and that was followed by the Army version in 1862. The Air Force used the Army and Navy variants until the creation of its own separate one in 1965. Since the retreat of the government of the United Commonwealth to the Antilles in 1921, it has been awarded by the Antillean-based United Commonwealth of America to its servicemen, while in the mainland, the United Commonwealth of Continental States (UCCS) has replaced the medal with various other decorations.

The awarding ceremony of the Medal consists of the president presenting it to the recipient at the National Assembly, as a symbol of gratitude from the Antillean people. According to the Antillean Armed Forces Historical Society, there have been a total of 3,221 medals awarded in total, and out of those nearly 50% were awarded during the American Civil War. A total of 95 medals were later revoked by the Federalist government in the Antilles as the recipients that earned them in the past had joined the Continental Revolutionary Army.
